Hello Parents and Guardians,
Screening to Care is a new program in your student’s school. The Screening to Care Program provides screening, social and emotional services, and referrals to Middle School students attending a San Diego Unified School District program. Schools receiving services are selected on an annual basis with the intention of serving all San Diego Unified School District Middle Schools over a five-year period.
A school-wide assessment of student’s social-emotional and behavioral needs is conducted using a self-administered brief questionnaire implemented by the sites served. These screening results are used to develop recommendations for school-wide workshops to build positive relationships between staff and students, as well as to identify students in need of social-emotional skill building/support, and/or behavioral health services.
We are sharing a link to a permission slip for your student to participate in the screener. Please review it and sign at your earliest convenience.

Screening to Care es un programa nuevo en el distrito. El programa Screening to Care proporciona servicios de detección, apoyo social y emocional, y basado en los datos brindados por la encuesta, Podemos referir a servicios adicionales. Las escuelas que reciben servicios se seleccionan anualmente con la intención de prestar servicios a todas las escuelas intermedias del Distrito Escolar Unificado de San Diego durante un período de cinco años.
Se lleva a cabo una evaluación a nivel escolar de las necesidades socioemocionales y de comportamiento de los estudiantes mediante un breve cuestionario autoadministrado implementado por MTM. Estos resultados de evaluación se utilizan para desarrollar recomendaciones para talleres en toda la escuela para construir relaciones positivas entre el personal y los estudiantes, así como para identificar a los estudiantes que necesitan desarrollo/apoyo de habilidades socioemocionales y/o servicios de salud conductual.
Compartimos un codigo QR para que usted de su permiso para que su estudiante participe en la evaluación. Revíselo y firme lo antes posible. Su estudiante recibira una copia del documento de permiso si prefiere escribir su firma en vez de hacerlo en la computadora.
